We are excited to announce that the Caro Athletic Department is launching FinalForms, a new online
system that will add convenience for families and help us boost compliance and safety for our athletes.
FinalForms allows you to complete and sign forms and submit your student's physical right from your
computer, tablet, or mobile device. Each student-athlete's parent or guardian will need to register with
FinalForms prior to the start of the 2024-2025 Middle School and High School athletic season or they
will not be eligible to compete.

The best news: FinalForms saves data from season-to-season and year-to-year, meaning that you will
never need to enter the same information twice! FinalForms also pre-populates information wherever
possible, saving you time when you have multiple children in Sports.

You are able to login at any time to make changes that include updating emergency medical contacts
and personal information. You will be required to sign your forms once per year and again any time you
make an update.

Please register Before August 1st at: https://caro-mi.finalforms.com/dashboard. First time registration
will take between 15 to 20 minutes.

Follow the prompts to create an account, manage your athletes, and sign all necessary forms. Click on
